On the evening of December 22, 2014, certain documents maintained by McDonald Eye Associates were discarded without being shredded as initially intended, some of which likely included protected health information (PHI) for some of our patients. We realized this error early in the morning on December 23, 2014, at which time we noticed the discarded documentation had already been removed by the City of Fayetteville Recycling and Trash Collection Division. Examples of PHI that could have been included were patient names, dates of birth, and Social Security Numbers. While we believe the risk of harm to be minimal, it would be prudent to monitor your financial and other accounts in the coming weeks to ensure no suspicious activity has occurred. McDonald Eye Associates takes the privacy and security of our patients seriously and discussed this issue with the responsible party at length, as well as reviewed the importance of timely shredding of documents containing PHI with our staff.
